Long and a bit hard to read, but good information .... Cliff Notes version, there were multiple failures of the system. A virtual perfect storm of wrong decisions, bad choices and, well, massive screw ups.

Lessons can be learned from mistakes. Unfortunately it takes a disaster before people realize that their status quo needs to be revamped. Too many snowflakes take advantage of disasters to reduce our ability to protect our children and the ones we love. WAKE UP AMERICA AND VOTE.
