That is most likely an institutional issue. If Admin stresses a policy enough on supervisors, then supervisors will be more apt to enforce that policy. It also comes down to how involved were the Admin and supervisory staff with their subordinates. I am not excusing the officer. They should have done their job, AND THEY SHOULD NEVER FALSIFY DOCUMENTATION.
