Had not seen this article LTC Ivan Raiklin, Esq.. Thanks for the situational heads up. Just curious but do you think errors like this are being caused by personnel fatigue (back to back deployments, under-staffing, increased ops, etc) and/or a general lack of enforcement of standards?
