He essentially shot himself by not following the rules regarding MRI scanners. It has been reported in the past of firearms discharging in an MRI scanner, thankfully no injuries in the other cases I have read about.

This is why they have all of the sign and ask if you have any metal, we unarm while bringing detainees into the outside room

SPC Robert Coventry if I had to provide anesthesia or sedation, I literally emptied my pockets. For a general aesthetic we had to really be inventive with jerry rigged tubing, etc.
