The ratings and evaluation system, although not perfect, is setup to vet individuals like this to prevent these from ever happening and before they become a blight on the service. I don't personally know either of these two or the situation, other than what's written. But personal experience is that there are always warning signs in their past that led up to this. Actions discounted, letters of reprimand removed (because someone's afraid it'll ruin their career), or whispers throughout the chain-of-command that were never taken seriously. I was a supervisor for one of these types; and when I attempted to write an appropriate evaluation, it was taken out of my hands (Sr Rater option) because I was told it would've ruined that person's career. The regulations are there; just need everyone to have a backbone and enforce the regs.
