Having done my fair share of investigations, I will say that they are always brutal. That being said... I weep for my community. I really don't know what is happening. I was always trained that when the sh@t hits the fan, regardless of circumstances, always be able and rely on basic seamanship. CBDR (Constant bearing decreasing range) is DANGEROUS! Do SOMETHING to change either the bearing problem or range problem, and you will BE OKAY. Most times it is nothing more simple as turning or changing speed. SN Greg Wright
Added: I certainly hope that there was bad weather because the incident happened at 1130. Not 0200 or when things get slightly more difficult. This was middle of the day, probably broad day light. UNBELIEVABLE!!!!

That is sad news in one sense that another failure has been identified however on the other hand it is hopeful that the investigation has been very thorough into the USS Lake Champion collision PO1 Tony Holland .
