Ok. I took your post to mean that you were not surprised that it happened. I agree that it is self-evident that it could only happen due to failures on multiple levels. The Swiss cheese effect, if you will.

Maybe now we can get back to standards and weeding out the weak military personel. This is not a game. I know people join because they have no other option or they want money for school or they saw the commercial and heard the music playing and they got all misty eyed or their parents were in the military...but this is a serious commitment. There is a chance you could die or get serious injured while serving. Our military needs to get it through these members heads that this is not a game at all. Train to standard and take it seriously or get out. No more fucking around and wasting tax payers dollars.

I don't think anyone with even basic understanding of military operations would've guessed any other possible outcome the real question is twofold specifically what was identified and what will be done about it .
With luck a single midgrade officer will be found as a scapegoat and then the rest of the issues forgotten about

SCPO Joshua I - Senior chief....what do you think? Commander was thinking "the Boss said go, Im not making the call to turn this off" and then on down the chain that same thought went?
The Navy seems to fire Commanders at the drop of a hat "loss of confidence" I wonder if that leaves a commander with "Well lets see If I fail the mission Im fired, if I dont do the mission Im fired, screw it what to I have to lose by sending them" mentality?

SGM Erik Marquez - No, that does not leave commanders with that mentality. This was a failure on multiple levels unquestionably. In my experience the low threshold for failure has had the opposite effect of more over-reporting than under-reporting, generally speaking.
