Leave it to human error to find a way to make a situation worse. Some reports suggest that the real cause for the emergency escalation was insufficient training for operators.

LTC Stephan Porter I have worked for the govt for 37 years. Most things they regulate have issues and nuclear plants are ran by human regulated by said govt. I do have friends in the industry, they do say the protocals are pretty solid. But ...there is always always Monday or a Friday in everyone's week.

LTC Stephan Porter - one of our engineers came from the Omaha plant- wicked smart. I have been doing a CPI project on communication within our Civil Engineering work control. They shirk checklists like the plague, but are seeing benefits to remove complacency, assign accountability, and to be honest a simple tool that verifies that the full spectrum has been completed. I worked MX for 16 years- we had protocols and checklists for every job. We still made mistakes. I would hope nuclear plant employees would pay more attention, but knowing some they are normal everyday people. I would like to see their training programs.
