I haven't read the reports, but I suspect an over reliance on navigation aids is the basic problem.

I think the 7th needs to do a stand down and re-evaluate their training program. While there are occasional accidents to have 5 + in one AOR in a year shows a serious flaw in the fleet. I know many Sailors love the "laid back" South Pacific location but this has entered a fatal situation. The complacency that has cost lives needs to be hammered in a violent and direct manner. I think to many in positions of authority have been covering for one another. Time to clean house and move personnel to far different locations.
