I would recommend the development of a CCIR matrix. It is constantly updated with actions, reactions, decision makers, and criteria for each.

One there are not any documented SOPs for my company so the way we do things can change on the whim of any of the company officers, and often does. Two I have transitioned from being an employee to a contract "advisor" so any information I provide has to be weighed by others before a decision can be made. When lines of communication fail, decisions can take longer than they should. When egos get in the way, those decisions may not come at all.
