I worked in manufacturing for a while. The problem I saw was excessive competition between shifts. Each shift wanted to out perform the other. They would cut corners to exceed the rate and their competitors.

I remember that when I worked at Pratt & Whitney Aircraft, they would actually have tally boards on what the previous shift had done.
