Walk up to your Finance office and ask them what the form should look like. That way you know it will be accepted the first time

Honestly, it probably won't matter much. Once you sign out, your OHA will stop and you will receive BAH Non-Locality rate until your ETS date. This rate is the same regardless of location, but the best answer is check with finance.

BAH is based off of where you are stationed, not where your spouse lives or where you are taking leave. You are still assigned to your unit in Korea until your ETS date.

Tacking that portion 1SG, finances' guidance was to complete a 5960 for my leave because of our housing being oha out here based on our rent. Im going to take a trip over there again in the morning for more guidance.
