It's pretty f***ed up when mistakes that big are made, resulting in multiple deaths, and the response is "oops, better fix this" with no other accountability.

This is a disgusting revelation about which nothing will ever ‘be done ‘. No one whosoever will ever be held accountable at any level. A total system failure for which the AF/feds should have to compensate the victims families.
