The whole system is to blame IMHO. Every part of the system, the controllers, the pilots, the altimeters, etc were a giant cluster foxtrot.

Plenty of blame to go around. And I got to watch the "duck and dodge" in person for part of it.

A lot of 'it wasn't me's' going on in this.

While I was active in Germany, there were 3 Secondary Cal Labs. One summer the other 2 labs had to shutter their Barometric sections and personnel were sent for retraining. They failed a test from the Army's Primary Standards Lab. I missed the check by parts per trillion. For that summer I was responsible for every transfer teams altimeter and pitot static test set in the European theater. I put in a lot of overtime. I don't know anything about the radar altimeter mentioned in this article, but I like the idea.
