Once at state level, it goes through a cursory check for correctness at G1, goes through a JAG review which could be 3-6 months, sent to OTAG for approval (could be 2-8 weeks for that), then signed by TAG or the G1.
Typically it is not "sent back" to the unit, it sits as an action on a G1 SharePoint or Teams tracker and changes to "complete", and the unit downloads it.

CPT Lawrence Cable I’ve been told that the paperwork is pending approval per the tracker so although I’m prepared for it if it were to be denied there’s a high chance that it will not be according to my leadership and previous members who’ve jus transitioned.

CPL (Join to see) - I know that some states have a policy of approving DD368's, Indiana National Guard I am told approves most if not all. That has not been the case in the past. States were I served were very reluctant to release anyone that hadn't served out most of their enlistment and about the only exceptions were hardships. The Guard has a pretty high investment in you by the time you complete IET, they want their money's worth.

CPT Lawrence Cable yea! I’m actually in the Indiana national guard lol and I have about 2.5 yrs left on my 6 yr contract too so hopefully they take that into consideration when reviewing my paperwork

CPT Lawrence Cable - Who knows? In my state, even a flag lift for APFT or HT-WT gets a JAG review.

I Don’t Know About Now But in 1998 I Went From Guard To Active, and The 368 Took 7 Months I am Guessing Same or Longer Now.
