How do I fill out a DA FORM 7652?

I am being med boarded due to 2 back surgeries with one ending in a spinal fusion. I was told that I need to put together my IDES packet to turn in to start the process. I've filled out everything I feel comfortable filling out but now I have to fill out this DA FORM 7652 (Disability Evaluation System Commander's Performance and Functional Statement. I don't want to mess this up. I don't know why my CO can't do this. I don't have the ability to properly word this document. I'm on a permanent profile due to the fusion and sciatic nerve damage. Could someone please help or point me in the right direction? It would be greatly appreciated.
