Hello all, Army guy here. Apologies for asking such a basic question, I've Googled around and didn't see anything that resembles what I'm looking for. The Army has a DA Form 705 that records the Fitness Test results for an individual (Push Ups, Sit Ups, 2 Mile Run) and a DA 5500 to record the body fat results.

What is this for the Navy? I'm looking at the Navy PRT Guide but only see tables to calculate scores. Thanks!

While the final results are recorded in PRIMS, I'll go forward with sharing a link to the NPC page that also links score sheets, http://www.public.navy.mil/bupers-npc/support/21st_Century_Sailor/physical/Pages/default2.aspx

The scoring is based on age group and gender. There are categories "Satisfactory" "Good" "Excellent" "Outstanding" and "Maximum". In example; I'm 30 (well, going to be) year old female. If I'm wanting a score of "Excellent" I would need to accomplish (at the minimum) 81 sit-ups, 35 pushups and have a run time of 13:30, (burn of 145 calories for bike and elliptical) (10:30 for 500m swim).

Got it! Thanks much. We used to have to drop our dog tag at the mile turn around. Popsicle sticks sounds easier.

MAJ (Join to see) - Not a problem, Sir! I run like an elephant stomping through peanut butter but dammit, I do it! I'm more on the bike since it's a bit easier on my knee. Plus I feel that I'm better a short bursts than long standing circuits.

I preferred the swim. Easier on the joints and easier with good form. As a plus the fitness coordinator hated it. Most times there were only a couple of us. I used to do the emersion test too. It dropped my body fat % by around 4%. It really pissed him off. They put me in the outstanding category.

Sir, do you have a Navy IA person attached to you or are they on Permanent duty assigned to your command. If they are assigned they should have a officer that is certified to conduct the test.
