Do you need a profile to wear a compression sleeve in the Army physical fitness uniform?

I would say yes, as it is not an authorized uniform article

12–2. Composition
The IPFU consists of the following components (see figs 12–1 and 12–2 for variations of the IPFU):
a.Jacket, running, gray and black.
b.Pants, running, black.
c.Trunks, running, black, moisture-wicking.
d.T-shirt, gray, short sleeve, moisture-wicking.
e.T-shirt, gray, long sleeve, moisture-wicking.
f.Footwear.
g.Socks.

12–3. Accessories
The following accessories are normally worn with the IPFU.
a.Gloves.
b.Fleece cap, foliage green (see para 4–10c).

I'm going to say yes since a compression sleeve can be viewed as a medical device.

Well that's what I'm saying 1SG (Join to see) , since normally items such as compression sleeves are not allowed, if SSG (Join to see) would like to wear one, he would probably need a profile stating that his doctor needs him to wear it based on a physical injury.
