Application for Active Duty Form 160-r questions

Hello, I'm getting my warrant packet together for 153a, and everything is going smoothly but im confused with DD Form 160-r Application for Active Duty. Im currently serving on active duty as 91H. I will list the areas I'm confused about.
Blocks 4a, 4b, 4c, 5a, 5b: do i put any of my current information? such as my branch is US Army, mos is 91H, component is regular army. OR do I enter information as if applying for 153a on reserve duty?
block 10: what is the difference between 'duty assignment' and 'area assignment'?
and is block 11 optional? im not really understanding what it's asking my preference on.
That is all my questions, hope i can get some answers! Thanks to anyone who helps in advance.

To respond to your question:
Blocks 4a, 4b, 4c apply only if you are reserve officer already, 5a and 5b do apply to your CURRENT MOS and service component. Block 10 is 153A for flight for assignment and then area is a particular location preference (post or country). Block number 11 is ASAP for all choices you make, so basically the form looks much like the example given in the recruiting packet which you should have a copy of.
Good luck in your application process, it is a little bit easier and I am not making that up, it was done with a typewriter and correction tape when I first applied. Digitalization has increased your ability to influence your packet quickly, the process used to take up to a year to get together which usually resulted in a expired flight physical.

Perfect. Thank you for your response, Chief. It is very much appreciated. And yes I have heard of the difficulties of putting a packet together with a typewriter. We have it easy now with the Adobe files.

Wow. I have never seen a PV2 put in a packet for WO. I know there isn't much really stopping you but were you able to get a letter of recommendation? I have never seen a person become a WO pilot that wasn't an NCO already. Please keep us abreast on the outcome.

I've been told if flying is what I really want to do then to submit a packet as soon as I qualify. Since my military experience is limited I'll be counting on my SIFT score along with some good letters of rec. I love helicopters and wish I knew I could've applied in my enrollment process but hey.

Aviation Warrants pre-qualifications are different requirements than all the others. If your scores are high enough and you pass the physical, you can actually come in and go straight to WOCS.

SSG (Join to see) I agree with you on that. I have seen a soldier get turned down due to he age. He went to a board and was well qualified. What I would be worried about is if a board would let a 19 year old soldier with a year in enter a WOCS program and fly at that age. It isn't that it can't happen but I have never seen or heard of a PV2 or PFC do that in any field.

Sir, I was a PFC when I submitted my initial request. Just FYSA, aviators are able to be born "street to seat" without any military background or education requirements. I will state that I was a SSG when I finally received my selection notice, and that was not the norm for flight. Technical warrant fields often are the only ones requiring stripes to be considered.
