Does a counseling for an NCO have to be done in writing in order to be put onto an NCOER?

I was never given any initial counseling, quarterly counseling or even knew who was my rater what my duty description was or anything. I have been trying to find it in th reg and pam but no luck. The verbal counseling was done with everyone in the same room

No, you don't have to be counseled on record for your accomplishments to be placed on your NCOER. If there are negative remarks on your NCOER those must be substantiated with recorded counseling or documentation of some type. Your unit is required to post the rating scheme, so ask your 1SG who your designated rater is. Your duty description is in the DA PAM 611-21. Take the description for the rank of your position, add the size of the unit you support and the number of Soldiers you are in charge of, that's your duty position

You don't have to but lack of all that will make an easy rebuttal of the NCOER.
