Does anyone have a template for a commander's memo authorizing a flight physical?

I no longer have the memo I used in Fort Campbell and now need one for a physical at Fort Belvoir. I'm stationed at Fort Meade so nobody knows what I'm talking about.

Managed to get in touch with one of my buddies who still had his, so the keypoints for this is that it needs to include that you are free of administrative action and that your command supports you getting a flight physical.

Some of the Air Force Types on here may have it but I'm doubtful that any of us Army guys could help.

Now that I think about it, wouldn't that be in your 201 file. It surely follows you wherever your duty station is.

I'll double check. Didn't really look like it was actually part of the physical last time, they kinda just glanced at it to make sure my commander and I were on the same page

Usually, if you go to your designated health clinic to the physical exam section they'll have a "fill in the black" memo. You just have to fill it out and bring it up to the Command team using the proper channels.
