I believe the first hour glass comes at 20 years. The ribbon represents the first ten years.

CPT(P) (Join to see) - negative. The Armed Forces Reserve Medal is never worn without a device. The award comes with the hourglass for ten years. One can also receive the award prior to 10 years for mobilizing, in which case the M device would be worn, and yes, both devices are worn for those who've earned it for mobilizing and for time in service.

I thought the ribbon wasn't awarded alone. Either you get it with a mobilization anytime prior to 10 years and there is an M device and then hour glasses for every 10. The device is the WHY they got it.
