Ran out of airspeed, altitude, and inspiration, all at the same time.
That is generally the cause.

I'm asking about Class A only and the ones with M/R #2 strap pack failure. The Apache that went down on Dec 28, 2016, my nephew, Lucas Lowe, was one of the pilots.
