I'm asking about Class A only and the ones with M/R #2 strap pack failure. The Apache that went down on Dec 28, 2016, my nephew, Lucas Lowe, was one of the pilots.

Are you inquiring about just class A incidents with injury and/or loss of life or class A-C? Just US aircraft or all?

The only one that i'm finding is the ANG Apache that went down during training in Galveston on 28Dec2016
