Asia, North America and Europe. To be fair though, I've been to Asia more than 40 times and make a point to go at least once per year, every year.

By convention there are seven continents: Asia, Africa, North America, South America, Europe, Australia, and Antarctica. Some geographers list only six continents, combining Europe and Asia into Eurasia.
Depending on which convention you use. I missed Africa (sad, but true) and the Antarctica (most of my undersea ventures were the top of the world). All others I have visited as military or civilian.

Some islands are part of some continents, it depends on what continental shelf they are on, Australia and Antarctica have their own, smaller islands that do not connect to a "shelf" are just lucky enough to poke out of the water. I'm not sure how we account for them, there are many in the Pacific.
