They should have started a MEB on you (medical folks). Was it a running profile? If it was I don't see how your APFT failure is blamed on you. Things are not adding up. You need to get a hold of a PEBLO (Physical Evaluation Board Liaison Officer) ASAP to discuss a MEB, and S1 about postponing your final out. But I am emphatic these need to be done by yesterday.

I concur with the good CPT. You should go to your Dr and ask about an MEB if your injury was the result of your active duty. Do not do yourself the disservice of not seeking medical advice on the way out. Have you completed SFL-TAP (ACAP)? Have you already had your "ETS" Physical? Contact your local PEBLO, as the CPT said. You should be focused on you over the next month and a few days. The MEB could/should extend your time in service in order to complete the process, but you shouldn't short change yourself.

Unless you have had an MEB, you do not have a P3. It doesn't turn into a "P" until that Board makes it official. If you have a T3, That's a Temp thing that you may have to fight.

But Whoa! A P3? Service Connected? That's a WHOLE 'Nother can of worms. If you have a P3 (or a P4) the MEB paperwork should say if it's Service Connected. Go to the ACAP office NOW and explain your situation. You could be looking at a monthly Medical VA check. Bring the MEB results with you along with any other supporting documentation. The ETS does NOT mean you are kicked to the curb. If your PULHES was a "Picket Fence" when you joined and now you have a P3, you have a case for VA treatment.

At best, they might withhold your leave. Your legal office, most likely, would advice the chain of command to let it go. Either way is an honorable discharge and not worth anyone's time to do a Chapter 13 (Unsatisfactory Performance) over a Chapter 4 (ETS or Retirement).

I will guess you are separating and not retiring.
I don't know your whole military record, so I will make my guess as each branch does their own thing.
Assuming that you had good reports and no discipline issues, a general discharge or honorable discharge would be in order.
However if you have negative reports or discipline issues, a general discharge would be in order.
You will most likely get a LOC or LOR for the failure for not keeping within standards this will be for record to "shut the door" and make it difficult to come back into the military at a later date.

It doesn't make a whole lot of sense to chapter someone that close to their ETS. But either way nothing will change as far as benefits unless you have over 6 years of service. Then you would be entitled to severance pay. Your RE code will be a 3 and not a one which will prevent you from joining again unless the service is really hurting for bodies. Once your chapter hits transisition that's when your leave will come into play. Make sure you inform your command you want to take leave or you will have to sell it. Once they cut your orders that's it.

Thank you for the information Fist Sergeant. I brought it up to my chain of command and they sent my Leave form up. They told me I just have to sit and wait till my leave date then I will know. Can I be informed sooner so that I may play my transition smoothly instead of last minute, or do I really have to wait till the day my terminal leave starts?

No, the Army is going to ruin your discharge because you failed a PT test right at the end of your enlistment.
