A printable version of AF Form 1288 is available on line. The endorsement areas for 1st, 2nd and third are things You do not fill out, those are done ONLY by the officials that handle Your application, NOT YOU.

I just noticed that SSgt Cadmus also addressed that issue, He is correct !

No need to fill out the endorsements section on the back of the form. The first endorsement is for your commander to sign. Once you're approved for being gained to the Reserves, the gaining unit commander will sign the 2nd endorsement. If you have a recruiter, they should tell you this and walk you through the process.

I'm an IMA applying for an AGR position. My active duty commander does not know who the first endorsement should be addressed to. Would that be my HQ RIO Det? Or would that be to the Commander of the posted position?
