If my commander never filed my LOD correctly from 2015 what can I do now to be able to get it filed?

Back in 2015 we were doing PT and I tried to avoid a pot-hole and still ended up rolling my ankle. I went to the hospital a bunch of times for it and my unit did the paperwork to start the LOD but I guess never completed it. I’m lost because I don’t know what to do with a sheet of paper of my commander stating it should be approve because of no misconduct but it’s 5 years later. I was trying to work with someone from AFSC / Magellan Federal And still never accomplished anything.
