General of the Armies of the United States. Made posthumously for George Washington, so he would always have the highest grade.

General Pershing also held that title. Washington's appointment was a posthumous one made by President Ford in 1976. The President said that this was being done so that Washington would rank first among all other officers of the Army, past and present.

Well, there is 5 star and actually 6 star General Officer Rank. Not certain but believe Pershing was awarded 6 stars. Washington was given 7 stars.

Technically, LTC, Pershing's rank is only equivalent to a 6 star. Following WW2 there were talks to implement 6 stars, officially. As it stands now, the rank is 4 gold stars.

No. You're thinking of Eisenhower, who was named Supreme Allied Commander of the Allied Expeditionary Forces during World War 2.

Don't know, but I have known a few candidates for Lance Corporal of the Marine Corps.

This was a basic training question when I went through, along with "Who was the last one?". This is not counting the Honorary rank of "General of the Armies" granted to Pershing and Washington, however. Only active military rank.
