Please clarify.
Had this service member completed MEPS, basic and MOS training? Active, Reserve?

Acknowledged. Understand MEPS, but have taken several physicals over the years and was able to get those at duty station, but status and assignment location dictate. Matters not much where, though. The Service and DoD regulations would apply with regard to med board, separation, etc., and whether member is considered disabled and not able to complete service, no longer eligible for reclass (if that's what purpose of physical was), and whether completed initial enlistment. Of course, whether AD or reserve. Not enough information to reference regulation.

In obligated enlistment, 4 more years, in selective reserve status, although UIC Unit Identification Code of SM is the Active Duty Command, he is embedded as augment of this operational Active Duty Command(in this instance FFG Guided Missle Frigate). Branch Transfer Chit Processed. MEPS Physical Ensues. Permanent Profile for multiple medical reasons results.

By branch transfer, do you mean transfer to another military branch, or transfer to another occupational specialty?
Does returned to unit, mean the transfer was cancelled?
Does the permanent profile prohibit member from performing (remaining qualified for) either/both positions?
Here are a couple of links to Navy medical board pages.

As long as he did a good job as a recruiter he should get promoted and reclassified if in combat arms.
