Where can I find an example of a 4856 for Soldiers to PT on their own?

PT on own, does anybody have an example of a 4856 that i could send up. It was a thing before we deployed and upon coming back i have soldiers that have no need to be subjected to crappy group PT and just want their requests to be in official writing. Thanks in advance

A 4856 is a counseling, not a request. It's a record of what you say to your soldiers.
If you're asking to do PT on your own, walk up to the 1SG and ask them. Chances are your unit/Battery/BN/BDE/DIV has a PT policy in place already and that's what you have to work within. When I was at the 82nd, absolutely no one was authorized to PT on their own, from the Division commander and CSM down. When I worked at SF Group, you worked PT into your schedule in accordance with your shop, team, or section mission requirements. When I worked at Ranger School you could PT on your own if you got a 290 or above.
Sending paperwork to some imaginary place isn't the answer. PT is a leader led activity, so you need to ask you leaders about it.

SGT (Join to see) so, you're not asking for Soldiers to do PT on their own, you're asking to do PT with your Soldiers.
Just drag their asses to PT. You're their NCO, that's your prerogative. You may have to submit a PT program, no one is going to say an NCO can't conduct additional PT.

Im requesting we do our own thing on our own time and not be subject to doing minimum pace runs in formation. For the unmotivated kids to not drag down my guys that want to do harder stuff with me. And i want to send it in an actual request because my chain of command doesnt like to do things the easy way of me asking and receiving an answer. If i get an answer one week the next something happens and they act like it never happened. I promise it isnt my first attempt to fix this, just trying to make an official attempt now.

SGT (Join to see) there is no 4187 for that kind of thing. 4187 is a request for personnel action. It is not a request for a training plan.
Who do you imagine it is that your request is going to go to when you I trying to make it official? Whoever that person is, walk up to them and ask them. Then tell them, you will submit a training plan and ask for their approval. That is the short and sweet version of how training is approved in the army.
A 4187 is for asking for an administrative action for an individual.

If your unit pt is crappy maybe you should do something about it SGT. If people arent getting anything out of it what good is it? Easy pt wont do you any favors in combat and it wont build camaraderie. You dont have to lead it, you can build a plan and let the SPCs lead it, they need the know how for BLC anyway.
I've never seen any paperwork to send up to request individual pt. Unit fitness is a commander responsibility so they will be the one dictating policy on when and how pt plans should be constructed and performed. Probably just need to talk with them about it and get it put in an official MFR. If you want it to be successful you will need to put some thought into who would qua jm ify for such a program and who will be responsible for keeping accountability of people in it.

Just alot of individuals that frankly do not care to participate in any decent PT so we get sucked into training at the lowest persons level. Thanks though!
