Given all of the electronic rotating and phased array / optical / sonar based air / surface / submersible surveillance systems - not to mention human lookouts ordinarily posted high above the water - on board the USS Fitzgerald - do we know why this collision occurred? Massive waves? Operator error? Design defect? Something else? Warmest Regards, Sandy :)
