One of the basic fundamentals of being a leader is understanding that you are responsible for everything that happens or fails to happen. No presidential nominee goes through confirmation without the approval of the nominating authority. If he didn't personally vet the "recommendation" his staff gave him, then he deserves all the heat. Then he can go fire everyone involved for his own mistake.

That's a leadership issue. If he isn't vetting them, or asking the right questions, then it's the process he's set up. One of the problems is other Presidents used an ABA Steering Committee to vet before they were moved forward. This administration chose not to do that.
