So Many errors by so many of the officers! A massive accident that might not have occurred had any one of those officers executed their jobs correctly...so sad!

and had anyone looked up from their screens, or out their windows (including the lookouts who should have seen and reported the hazard).

That sort of thing did not happen in the 60s. We transited the Inland Sea of Japan at night several times. The old man was on the bridge. We were around Tokyo Bay at night. The old man was on the bridge. I guess that was then and this is now.
