They should beat someone or someones over the head for this! Without data we don't know what went wrong, how to track trends, prevention, inspection cycles, etc. ARRRRGHGHGH

Accident investigations and Collateral investigations used to be two different things in my day. Accident investigations only tried to find preventative - not punitive solutions. Collateral boards dealt with the punitive. I'm not sure how things are today, but I suspect that the "data" may have become "skewed" because people are afraid for their careeers...
