Obviously, "off label" use is quite risky. A physician who prescribes it that way, needs to warrant it as "medically necessary".

My eye surgeon injected something in my eye that was off-label, but he explained it throughly first, and had a bunch of peer-reviewed studies to support it. He even offered to provide them to me to read, if I wanted, but I passed because I trusted him and he had evidence to support it. It worked great, too.
