As someone who lives in Spokane, the author did almost no research. There are some anti-vaxxers here, but not more than anywhere else. Plus 99% of people wear masks in public everywhere. Not to mention, they got didn't even Google the name of the medical school in the city. Washington State University and University of Washington are different universities.
