This is part 2 of the totally unsatisfactory chain of events that led to a pilots death. Now we find out that there was also a known problem with the ejection seat. This whole scenario is unacceptable on so many levels.

What really worries me was this paragraph:
""Leadership also reviewed the accident investigation board report and has taken multiple administrative actions to hold responsible individuals accountable," he said, adding that the administrative actions are covered by the privacy act and cannot be released."
This was a major failure of standards in way too many arenas!
