How?! Dry fire means no ammo at all. Not even blanks. Rounds shouldn't have even been issued at this point!
Ordinarily I'm not big on command lynchings, but if a Commander ordered that live rounds be distributed before a dry fire exercise, he does actually need to be fired.
Any senior NCO could have told you how bad an idea that is...

Somebodies head needs to roll... Some body needs to set an example for such and oversight... What a sad day, the man had a pregnant wife....

This leads me to believe that other safety measures were not taken. Where was the marine when he was killed? Down range? Firing line?

Words fail me. But where the hell were the Lt.s and the NCOs brains who went along with this??? When weapons came out, I always checked weapons and magazines.
