Cannot put all the blame on leadership. Leaders could employ every risk mitigation possible and there will still be heat cats. Ultimately it is an individual responsibility for SMs to take care of their bodies. This is another side effect of kids growing up indoors and mashing buttons on a game console. They dont have a full understanding of what their bodies are capable of.
Where the military really messes up is they treat cold weather injuries as a huge deal, but only one cold weather injury will kill you. No one blinks an eye at a heat injury but, if untreated, all heat injuries can result in death. They've got it back wards.
