From article: "David Lochridge, himself an experienced submarine pilot, alleged in a 2018 lawsuit that he was fired after raising concerns that the company wasn't properly testing the vessel's carbon fiber hull. He had also pushed for the company to utilize a classification agency to inspect and certify it."

Innovation without building for long term safe operations is a recipe for disaster. Sounds like they pushed the envelope too far for too long and the day finally came where the issue became catastrophic.
As a submariner, I'm appalled by the lack of back up controls, manual controls, emergency beacons. All just common sense items that anybody who knows anything about working undersea knows.
I'm betting that the entire industry gets a subsafe type of program and that the cost of doing this business pushes a lot of companies out of this sphere.
