As a reminder, Robert Card was an Army Reservist who killed 18 people in Lewiston Maine last year. His friends and fellow soldiers flagged his increasingly paranoid and erratic behavior to authorities but nothing was properly followed through with.

Unfortunately the medical and law enforcement entities are more reactive than proactive.
