What Kim wants is to take over the Korean peninsula as a Communist dictator. He knows he cannot do that with the U.S. forces in place. So, of course he wants us to leave. En route to that, he doesn't want us to train with the ROK forces. So he demands we stop any/all exercises on the peninsula. The DPRK could get food and medical help in days if it abandoned its nuclear weapons and missile developments. But they won't, not ever. Kim cannot back down or his own military would probably take him out. Tough problem.
