Collision due to lack of basic seamanship and command oversight. Never cross a ship's bow. Don't change lanes in a Traffic Separation Scheme. Rule 10 of COLREGS, "(c) A vessel shall, so far as practicable, avoid crossing traffic lanes but if obliged to do so shall cross on a heading as nearly as practicable at right angles to the general direction of traffic flow."

In in narrow channels or TSS, we set the Special Sea Detail with one anchor at water's edge. This put more eyeballs in action, had after steering manned, and had the CO and Ops on the bridge.

I'm curious as to what your thoughts are, considering this graphic. You figure they got the contact mixed up with another track, or just didn't have eyeballs on it ever?

Thank you SN Wright. I think all of us "hope" we'd have done better...and can't resist thinking of how we might have. Incidents like this often have me recalling "close calls", and being increasingly thankful for "luck"...which is anything but.

LCDR (Join to see) - You can always call down to Central Control Station to get more speed. Response time is very, very quick. If I had to guess (and I went to CHENG school with the CO of McCain), they had inexperienced helm/lee helm operators, and when the transfer problem occurred they all got their heads buried in the problem. Someone should have kept their heads out of it and focused on what was going on around them, but I wasn't there. Every ship I was on, we had modified S&A at least in TSS where there was a lot of traffic, not sure why they didn't...
