Not uncommon at all. Though modern carriers have more height and longer runway. I have seen many a plane drop lower than the bow on Launch. And 1 never came back up. Though pilots ejected before we ran over the plane. Cold CAT is a real thing
Only once in my 3 tours did we ever launch an F14 cold. Without using the shuttles to kick them up to speed. Definitely not for the faint of heart

I would say a fully loaded B-25 with no cat was pretty unusual. Those crews had brass ones the size of cannon balls.
