For those on active duty, what are the fields on your dog tags?

I will be using this for an article. My old dog tag had name, SSN, Blood Type, and Religion. What is on yours, and what is the date it was issued? Are you on active duty and will pass an inspection with those dog tags?

These days it's Name, DODID Number (instead of the SSN), Blood Type, Religious Preference. I got this set last year I think, when we switched from SSN to DODID.

Not at all, the DODID Number just stated getting used more in the last year or so, gradually replacing the SSN on most things in an effort to better protect PII. I'm not sure if Soldiers are using it in Basic/AIT yet, it's been 16 or so years since I went through.
Most serving troops I know are still looking at the back of their ID Card to remember their number, but people are getting more used to it.

Mine were issued with: name, blood type, ssn (but now it's dod#), and religious preference.

I was issued a pair in Basic during Reception with my Name, SSN, Blood Type, and Religion.
I got another pair with the SSN replaced with my DODID Number.
I'm National Guard but same issued tags as my AD brothers in IET.
