In World War 2 there were several. Generals of the Army Douglas MacArthur, Dwight Eisenhower, Omar Bradley, George C. Marshall and Henry H. "Hap" Arnold (also, later, the only General of the Air Force.) Also Fleet Admirals William Halsey, Chester Nimitz and Ernest King.

4 star, "peace" time.
5 wartime, but I'm told perishing could be regarded as a 6 star.

Just read a bit about this. In 1976, Gerald Ford promoted George Washington to the created rank of "General of the Armies of the United States," as the blurb said, so that he'd be the highest ranking officer, no matter what the situation. This blurb said that this rank was, essentially, a 6 star rank.
