I currently have found out about the drivers badge, and I do qualify. If my research is correct, I have to fill out a DA 4187 and submit a DA 384-E (Drivers Record) and 5984-E (Operators Card) . What else do I submit? Is there a form or something that I can put down my miles? Does anyone have examples on what they sent?
Thanks in advance, no one from my unit knows about it and Id like to know the process for myself and to assist other soldiers.

Any commander at the LTC/O5 or higher can award the deiver’s badge. Once the Soldier meets the requirements per AR 600-8-200, par 8-20; the orders issuing authorities just publish whatever (orders, 4187, etc...). Just need the unit’s master driver to endorse and submit thru appropriate channel the packet.

I just received this award. Per the regulation that others have provided to you, you only need a memo with your name, rank, social security number and the memo should specify its for the Drivers Badge. I believe it was sent up to my BC and he signed off on it. After that I took it to my S1.

You need either validation of miles driven or have had your military driver's licenses for a year with no issues, from my understanding of the regulation. So don't let them try to mess you over saying you need back date records of every single mile driven.
