Classic study in Engineering Ethics. The failure occurred because the manufacturer of the walkway supports objected to the design and proposed a different design. The responsible engineers in the architectural/engineering firm approved the change without reviewing it. Neither did the construction manager question it and he should have since the revised drawing had no stamp or revised calculations. The fundamental flaw was that the original design had both of the walkways self supporting. In the revised design, the fourth floor walkway had to support the weight of the second floor walkway. This wasn't accounted for in the revised design and no one, especially the responsible professionals, questioned it. 114 people died. All of the architects and engineers involved lost their license to practice.
