This isn't just a VA problem - it seems to be endemic to civilian nursing homes and other care facilities. My service widow mother was in nursing home/hospice for a few weeks before she died. Because of my work schedule I only saw her every couple of days. One day I entered her room and as I rounded her bed I saw a huge wet spot on the rug beneath her catheter bag which was bulging with reddish urine. On another occasion she had wrapped her oxygen canola tubing around her neck multiple times and was struggling to breathe - she had dementia. The hospice staff were all in the lounge playing a board game and ignoring multiple buzzers at the nursing desk just across from them. One can hardly blame minimum wage employees without adequate supervision, but it was troubling, nonetheless.

I just posted an article about the VA Secretary favored by some vets and not favored by some vets. This is another disgrace by the VA system. If I was this mans family, I would be beside myself with anger.

Anyway, I think you're correct about who should investigate this, if it is investigated.
