MAJ Dale E. Wilson, Ph.D. - Ballots need a serial number. That number would be stored with the vote , and only acceoted if it is both valid, and only used once. No records kept of who was issued the ballot, other than that the person voted with proper ID before being handed their ballot.
Add an 'ACCEPT' button to the machine that reads your ballot, to allow you to review it before you leave your polling place.
